concat patch for Buffer in node < 0.8.

Node.js 0.8.0 and newer provides this functionality out of the box on the
native buffer object (
Buffer.concat). If you rather this module work as a
polyfill, you can require
it as such:
require('buffer-concat/polyfill'). Doing so will make sure that
Buffer.concat works no matter what version of Node.js you are using.
